Office Policies


 

Due to the high risk associated with a pregnant woman's exposure to childhood
illnesses, we ask that all children 13 years old and younger not be brought to the office
during your visit--unless they are registered patients of the practice with a scheduled appointment. Albeit, we welcome our newborns that return with Mothers for their
post-partum visit.
We regret any inconvenience that this may present to you. Thank you for your

OFFICE HOURS


Our office hours are 8:00 a.m. until 5:00p.m. Monday through Thursday and Friday
from 8:00 a.m. until 03:00 p.m. There are early morning appointments available upon
request, Monday through Wednesday, beginning at 7:30 a.m. Our phones are
personally answered by a member of our team weekdays, beginning at 8 a.m. until 4
p.m., with the exception of our lunch period and the modified Friday schedule.
When scheduling your appointment please give the reason for your visit. This helps us
better schedule the amount of time needed for the provider to evaluate your individual
situation. We make every effort to keep on schedule; however, the nature of our practice
may cause the need for the physician to be called out on an emergent situation.
Although this is a rare occasion, when this happens, the receptionist makes every effort
to call the patients beforehand. We will keep you updated on the status of the
physician’s arrival. We apologize in advance for any inconveniences this may cause.

Preparing for an appointment


For our new patients, after you schedule your appointment, you should receive an
email requesting that you join our patient portal. Please do so and update your medical
history online, prior to your arrival to our office for your appointment. Please reserve
15-20 minutes to complete your history so that you are able to be as complete and
detailed as possible. Completing your medical questionnaires online, prior, to your
appointment greatly reduces your in-office wait time. It also provides the physician the
opportunity to review your chart prior to meeting with you thus providing a more
comprehensive experience. Another option is to print the new patient packet, located
under the Patient Forms tab, complete the forms, and bring them with you on the day of
your appointment.
We ask that new patients who have completed their medical history online arrive 10
minutes prior to your first appointment. If you have not completed your history online,
please arrive at least 25 minutes prior to your first appointment.
For an established patient, you will receive a text message requesting that you follow
a link and complete an update. Please do so prior to your arrival at the office for your
appointment. Please arrive 10 minutes before your scheduled appointment time.
Every effort is made to keep your wait in the reception area to a minimum.
Please bring your insurance card and photo ID.
Please bring your co-pay or be prepared to pay for any co-insurances.
Payment is due at the time services are rendered. We accept:
Cash
Credit Cards (Visa, Master Card, Discover)
Amex is accepted for payments greater than $100

 

CANCELLING APPOINTMENTS


We make every effort to schedule our appointments so that each of our patient’s needs and concerns are
met. In order to quickly meet our patient’s needs, and to not overbook our schedule, we require a 24 hour
advance notice in the event that you need to cancel an appointment.
ALL PATIENTS THAT FAIL TO KEEP THEIR SCHEDULED APPOINTMENTS WILL BE CHARGED A
“NO SHOW” FEE OF $25 FOR ESTABLISHED PATIENTS AND $50 FOR NEW PATIENTS AND
CONSULTS. FOR ALL SURGERY PRE-OP AND SURGERIES THERE IS A $100 FEE.

 

AFTER HOURS

After our normal business hours, you may reach the doctor for emergency calls only by
calling 770-551-9616. If a return call has not been received within 30 minutes after
you’ve called the answering service, please call again and advise them this is your second
call. Please make sure that “call block” has been removed from your telephone.
TELEPHONE CALLS
Due to the nature of our practice, we handle a large volume of calls. We have nurses who
are trained to answer questions and advise patients. All urgent calls will be handled as
emergencies and may require the advice of the physician. Please allow time for our
nurses to consult with the physician before calling you back. All calls will be returned by
the end of our business day. Please make sure you provide all of your phone numbers.

 

PRESCRIPTION REFILLS

Please have your pharmacy call or fax our office for prescription refills. If you call us for
a refill, please have the pharmacy name, telephone number, and prescription name that
you are requesting. All routine prescriptions are called into the pharmacy within 48
hours. If it is a non-routine prescription, we make every effort to call it in by the end of
our business day. Please call your pharmacy to make sure that it is filled before you go to
pick it up. There will be no prescription refills called in to the pharmacy after hours.
We cannot prescribe new medications over the phone.
NOTIFICATION OF LAB RESULTS
If you call in for lab results please provide our staff will all the phone numbers where
you can be reached. Our nurses will return phone calls at the end of our business day.
Please allow ample time for labs and test results. Many tests require 48 hours to several
days before a result is known. Please allow 24 hours for our staff to return your call.
For all pap smears, our office will mail you a letter within three weeks of your visit for
normal results. If you are registered on our online portal, you will receive notification of
your results by an e-mail requesting that you log into the portal.
If your results are abnormal, a member of our staff will call you.
DISABILITY FORMS
Due to the rising costs in Healthcare and the increase in required paperwork from your
insurance providers, employers, etc. there is a $25 fee assessed to any patient requested
paperwork.
